WD - Assignment Change Request

Request Information

The Request Information section includes fields to enter the requester’s UT EID, a description, and request notes.

The requester is the person who creates the request form. By default, the user’s UT EID displays in the Requester UT EID field. To change this, type in a different UT EID or click the lookup icon lookup icon to search for an individual.

The Description is a free form field, which allows the requester to add notes up to 150 characters. The description can be edited by the requester or anyone to whom the form is assigned. Best practice is to include a meaningful description and will be useful in locating the form when a search is performed.  Note: Do not include confidential information in the description, as this information appears to anyone authorized to search on HR forms on the Search page.

The Request Notes is a free form field, which allows the requester to add notes up to 1000 characters.

Assignment Change Information

The Employee Information fields are used to enter one of the following: the employee UT EID or employee name. Only one of the two fields is required. Click the lookup icon lookup icon to search for an individual’s UT EID. If the person is in multiple positions, indicate which one the change is for.

Types of Changes

The Type of change checkboxes are used to select the types of changes being requested for the assignment. One or more boxes may be checked. Additional fields appear below for each box checked.

Change Working/Business Title

Note: Changing the Working/Business Title is available to the employee via Workday self-service functionality.

The Current Working/Business Title field should reflect the employee’s existing title.

The Proposed Effective Date field is required and should use a MM/DD/YYYY format.

Enter the employee’s new title in the New Working/Business Title field.

Include an explanation for the change in the Justification/Reason field.

Change Supervisor

The Proposed Effective Date field is required and should use a MM/DD/YYYY format.

List the name or UT EID of the employee’s current supervisor as well as their new supervisor’s name or UT EID.

If the new supervisor has more than one position, indicate which position the change is for.

Extend or Modify Job End Date

If the position is permanent, select the Remove End Date check. If the position is temporary,  enter a MM/DD/YYYY format date in the Proposed End Date field.

Include an explanation for the change in the Justification field.

Change Hours per Week (FTE)

Enter the employee’s weekly hours at present in the Current Hours per Week field.

Choose a Proposed Effective Begin Date for when the change in hours per week should go into effect.

Choose a Proposed Effective End Date if desired.

Specify the employee’s New Hours per Week

Include an explanation for the change in the Justification field.

Change Work Schedule

Choose a Proposed Effective Begin Date for when the change in work schedule should go into effect.

Choose a Proposed Effective End Date if desired.

Choose a new work schedule from the Select New Work Shift drop down.

Include an explanation for the change in the Justification field.

Unpaid Leave or Unpaid Time Off

The Type of Unpaid Leave or Unpaid Time Off field is used to select the type of leave without pay. Use the drop-down arrow to select the correct type.

The Days and Hours Details fields are used to enter the hours off per day and the begin and end dates the employee will be on leave without pay. More than one set of begin and end dates and hours per day can be entered. Click Add Row to enter additional line entries. Click close icon to remove a line entry.

Change Funding Source

Note: At least one of the following fields must be completed.

Free-form Describe the Funding is a check box that when selected, will open a free-form field where a description of how the job is to be funded can be entered.

Enter Details for Job Funding is a check box that when selected, will open several boxes below where you can enter a Time Block to indicate which accounts will be used to fund the position, and for what time periods. In the time block field, you can enter begin and end dates, an account number or description, and the funding percentage.  Department codes can also be entered by selecting the check box. Additional time blocks can be entered for additional time periods.

Click Add Account to enter additional time blocks. Click close icon to remove a time block.

Salary Adjustment

Start by choosing at least one of the three available check boxes: Free-form Describe the Funding Change, Adjust Base Salary (recurring), or Adjust Allowances or One Time Payments.

The Free-form Describe the Funding Change field requires a free-form entry describing the changes you would like to make.

The Adjust Base Salary (recurring) field allows a MM/DD/YYYY begin date, Adjustment Type chosen from a drop down menu, an Adjustment Amount given in either Dollar or Percent quantity, and a Justification.  Click Add Adjustment to make more than one recurring salary adjustment, and click close icon to remove a line entry.

The Adjust Allowances or One Time Payments field allows a MM/DD/YYYY begin and end date, Type chosen from a drop down menu, an Amount given in either Dollar or Percent quantity, and a Justification.  Click Add Adjustment to make more than one recurring salary adjustment, and click close icon to remove a line entry.

Change Work Location

Enter an Effective Date in DD/MM/YYYY format, the New location for the position, and a Reason for change.
