About the Request Form System
The Request Form System is a Web-based system, which allows university employees who do not have authorization for administrative systems to initiate a request. Requests can then be assigned to one or more employees who can take any number of actions including: update, approve, assign, decline, or finalize. When the request is finalized, it generates an electronic document in the appropriate system (e.g., *DEFINE) or an email to the appropriate office to notify the department of an action(s) that must be taken.
The Request Form System is designed to reduce duplication of effort by decreasing the amount of data entry required on the part of administrative staff and to increase the accuracy of request initiation.
